Marshfield Medical Center-Beaver Dam's Hillside Manor Transitional Care Unit (TCU) provides rehabilitative short-term care after hospitalizations or illness.

When your goal is to return home, the professional staff at Hillside Manor’s TCU will be there with you each step of the way.

The TCU is located within Hillside Manor and connected to Marshfield Medical Center-Beaver Dam making it very convenient for medical imaging, lab and other medical visits.

Your discharge plans begin immediately with the interdisciplinary team at Hillside Manor to create a personalized treatment plan. We’ll keep you and your family informed on your progress. Our therapists will work with you extensively so that you can meet your goals. Before you are discharged, you’ll have the opportunity to visit your home with your rehabilitative therapist to ensure a safe transition. And we’ll ensure that you have the equipment you’ll need and arrange for home supportive services or outpatient therapy services. We’ll check on you after you are home to ensure that you have correct medications and are receiving needed services and answer questions you may have.
