Accreditations and Licensure Across the Continuum

Marshfield Medical Center - Beaver Dam

Accreditation from The Joint Commission was received in May 2019 by demonstrating compliance with The Joint Commission’s national standards for healthcare quality and safety. To earn and maintain accreditation from The Joint Commission, an organization must undergo an on-site survey at least every three years.

Hillside Manor

Hillside Manor is licensed as a skilled nursing facility that accepts Medicare and Medical Assistance.

In May 2019, Hillside Manor received accreditation through The Joint Commission after an on-site survey by demonstrating compliance with The Joint Commission’s national standards for healthcare quality and safety.

Hillside Manor is also surveyed by the State of Wisconsin Bureau of Quality Assurance to assure that the facility meets all state and federal regulations. 

Hillside Home Care and Hospice

Hillside Home Care and Hospice are both licensed by the State of Wisconsin. To maintain this license, the agency must undergo periodic on-site survey. Hillside Home Care and Hospice was also accredited through The Joint Commission in May 2016.

Hillside Home Care has achieved Home Care Elite status 11 of the last 13 years, a recognition given to the most successful Medicare-certified home health care providers in the United States. 

Also, Medicare provides a Home Care Compare website where you can compare the quality and care of home care agencies in your area. 

Beaver Dam Reference Lab

The Accreditation Committee of the College of American Pathologists (CAP) reaccredited Marshfield Medical Center - Beaver Dam's Reference Laboratory based on results of an on-site inspection as part of the CAP's Accreditation Programs.

During the CAP accreditation process, which is designed to ensure the highest standard of care for all laboratory patients, inspectors examine the laboratory's records and quality control of procedures for the preceding two years. CAP inspectors also examine laboratory staff qualifications, equipment, facilities, safety program and record, and overall management.

The Beaver Dam Reference Lab is currently accredited through November 2020.

Eagle's Wings and Remembrance Home

Eagle's Wings and Remembrance Home Dementia Specific Community-Based Residential Facilities are licensed by the State of Wisconsin. To maintain these licenses, each facility must undergo an on-site survey to assure compliance with all State regulations. At the last inspection, the Bureau of Quality Assurance issued no citations and determined the facilities to be in full compliance with state regulations.

Kids Care

Kids Care is licensed by the State of Wisconsin. To maintain this license, the day care center must undergo several licensing visits each year to assure compliance with all regulations of the Wisconsin Department of Health and Family Services.

MQSA Mammography Accreditation

The U.S. Department of Health and Human Services Food and Drug Administration (FDA) requires annual inspections regarding the Mammography Quality Standards Act (MQSA). In every facility providing mammography services this inspection reviews medical records and mammography reports, QC test results, continuing education of the Mammography technologists and radiologists, and digital mammography unit. Mammographic imaging services and digital equipment at BDCH has full American College of Radiology accreditation through Feb. 19, 2019.

Individually, Marshfield Medical Center - Beaver Dam is also accredited by the American College of Radiology (ACR) for two-dimensional and three-dimensional mammography through February 2022.
