Support for the Community

Four times per year, the BDCH Foundation accepts grant applications from non-profit organizations within the region requesting financial assistance for projects that improve safety, promote community wellness and provide education on health-related issues. Grant application forms are available here or by calling 920-887-4851 and are reviewed quarterly, with due dates of Jan. 15, April 15, July 15 and Oct. 15.

The Foundation’s Fund Allocation Committee reviews each application to ensure it aligns with Foundation objectives and then makes recommendations to the Foundation Board of Directors on which grants to fulfill.

Improving Safety and Quality

  • Breast Health diagnostic equipment, education and supplies
  • Funds for automated external defibrillators for Beaver Dam Police Department, Beaver Dam Unified School District, Society of St. Vincent De Paul of Dodge County, Dodge County Sheriff Department, Clyman Lions Club, Inc., City of Beaver Dam Community Activities and Services, St. John’s Lutheran Church and School, Grace Presbyterian Church, Green Valley Enterprises, Good Shepherd Lutheran Church, Beaver Dam Area Community Theatre, YMCA of Dodge County (Y Kids 2), Pillar & Vine, Inc., Town of Beaver Dam Police Department and Wayland Academy.
  • Funding to enhance quality-of-life activities at Hillside Manor and Remembrance Home
  • Funding to Dodge County Human Services for their “Cribs for Kids” program
  • Funding to the Randolph and Cambria-Friesland FFA to maintain food plots for local food banks
  • Provide partial funding to Mayville Parks and Recreation for playground equipment

Community Health

  • Partial funding of Horicon Jr./Sr. High School Fuel Up to Play 60 Program
  • City of Beaver Dam Community Activities and Services Arthritis and Strong Women Exercise Programs
  • Funding to local churches  to provide refreshments to Blood Drive participants
  • Funding to Lomira School District to purchase Medicine Balls and Storage Racks for the Phy Ed Department
  • Provide funding to Green Valley Enterprises for their Y5210 Healthy Choices Program
