Remembrance Home Dementia Specific CBRF is a community based residential facility specifically designed to meet the needs of persons with Alzheimer’s Disease and other related forms of dementia.

Located in a residential area in the city of Beaver Dam, Remembrance Home is state licensed for 19 beds. Keeping the needs of the residents foremost, the facility provides a safe, home-like environment with lighting and décor designed to be calming. Delicious meals are cooked on site and residents have easy access to an enclosed yard with a patio area and walking paths.

Each resident bedroom contains a twin bed, bedside table, dresser and chair. Each room also has private bathroom facilities, central heating and air conditioning, telephone and cable hook-up, window with a window seat, linens, plenty of closet space.
